  Disability:IN Utah in partnership with Goldman Sachs, Disability:IN Utah Business Partners and Community Partners are excited to announce the 2026 5K Run, Walk and Roll at Liberty Park on the morning of Saturday, June 27, 2026.
  
  

  Join us to Run, Walk or Roll the 5K route around Liberty Park or to network and meet the Disability:IN Utah team. 

  

  The registration form has a check box to mark you have read and agree to the Waiver and Release of Liability.  Please find the full text of the form below. A Google Doc link is also available on the registration page (please note that the Google Doc is not fillable).

  In consideration of being permitted to participate in the Disability:IN Utah, run, walk, roll 5k race (the "Event"), I, for myself, my heirs, executors, administrators, assigns, and personal representatives, agree to the following:

  
  

  1. Acknowledgment of Risks: I understand that participating in a 5k race is a potentially hazardous activity. I acknowledge that there are risks and dangers associated with participating in the Event, including, but not limited to:

  o Falls

  o Collisions with other participants, spectators, or objects

  o Uneven terrain

  o Obstacles

  o Weather conditions (e.g., heat, humidity, rain, cold)

  o Traffic

  o My own physical condition

  o Medical conditions

  o Dehydration

  o Exertion

  o Potential for injury, including minor injuries, serious injuries, permanent disability, and death.

  o Risks associated with travel to and from the event.

  
  

  2. Assumption of Risk: I freely accept and fully assume all such risks, dangers, and hazards, and the possibility of personal injury, death, property damage, or loss resulting therefrom. I acknowledge that I am physically fit and have sufficiently trained for this event.

  
  

  3. Release and Waiver: I hereby release, waive, discharge, and covenant not to sue Disability:IN Utah, run, walk, roll 5K, Disability:IN Utah, Goldman Sachs and all Community Partners, any volunteer, Salt Lake City and County and the State of Utah, and their respective officers, directors, employees, agents, successors, and assigns (collectively, the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of my property, whether caused by the negligence of the Released Parties or 

  or otherwise, while participating in the Event or while traveling to or from the Event.

  
  

  4.     Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in the Event.

  
  

  5.     Medical Treatment: I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during the Event.

  
  

  6.     Use of Name and Likeness: I grant permission to the Released Parties to use my name, image, likeness, voice, and any photographs, motion pictures, recordings, or other record of this Event for any legitimate purpose, including advertising and promotional purposes, without compensation to me.

  7.     Rules and Regulations: I agree to abide by all rules and regulations of the Event.

  
  

  8.     Severability: I agree that if any provision of this waiver and release is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

  
  

  9.     Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of the State of Utah.

  
  

  10.  Acknowledgment of Understanding: I have read this waiver and release of liability, fully understand its terms, and sign it voluntarily. I understand that I am giving up substantial rights, including my right to sue.

  
  

  If Participant is Under 18 Years of Age:

  
  

  I am the parent or legal guardian of the above-named minor. I have read this waiver and release of liability, fully understand its terms, and consent to the minor's participation in the Event. I hereby agree to the terms and conditions of this waiver and release on behalf of the minor.
